Confidential National Voluntary Event Reporting Tool (CONVERT)

CONVERT is the initiative of CAHO to create the repository of adverse events and "Never Events" which are a great learning resource for healthcare professionals and a huge step for improving patient safety. The event should be ANONYMOUS and can be in the form of storyboard.

CAHO invites its members to contribute to the repository.
